This full-time, salaried position ($44,000–$50,000 annually) offers remote work, health coverage, 401(k), PTO, and holiday pay. Our Remote Medical Assistants and LPNs (“Care Coaches”) engage chronically ill patients in meaningful monthly consultations to improve outcomes and promote self-management. We are now seeking an experienced LPN Team Lead to supervise and coach this team, ensuring consistency, compliance, and productivity while maintaining a supportive environment.

Job Summary

As an LPN Team Lead, you will oversee a group of Remote Medical Assistants and LPNs, ensuring that team members meet productivity, documentation, and attendance requirements while adhering to company policies. You will act as both a mentor and a working “Floating Care Coach,” stepping in to support patient calls when needed. You will also monitor performance metrics, provide coaching, and escalate issues appropriately—all with the goal of delivering consistent, high-quality patient care.

Key Responsibilities

Policy Enforcement

  • Ensure all team members follow company policies and procedures.
  • Address non-compliance or policy violations promptly.
  • Clearly communicate policy updates to the team.
  • Maintain a call census of typically not more than 100 patients per month.
  • Act as a Floating Care Coach when needed, recording notes and billable time for abnormal results escalated from the RN support team.

Productivity Monitoring

  • Review productivity reports three times daily to assess team performance.
  • Verify each team member completes at least 30 patient calls per day.
  • Ensure that 80–100% of the team's work time is billable and linked to patient activity.
  • Identify underperformers (below 80%) or overperformers (above 100%) and take corrective action, including coaching and training.
  • Promote consistent performance within expected standards.

Attendance Oversight

  • Monitor attendance and ensure compliance with company policies.
  • Address attendance concerns in a timely manner following disciplinary procedures.

Team Leadership

  • Set clear expectations for team performance and behavior.
  • Provide guidance, feedback, and support to drive team success.
  • Foster collaboration and motivation aligned with company goals.

Reporting & Communication

  • Analyze daily productivity reports to identify trends and opportunities for improvement.
  • Communicate findings to management and implement improvement strategies.
  • Submit End of Day Reports (accomplishments, challenges, staff issues, and red flags).

Qualifications

  • Current LPN license (active and in good standing).
  • At least 3 years of clinical experience with 1+ year in a supervisory, lead, or training capacity.
  • Proficient in medical terminology, vital signs, and EMR documentation.
  • Strong organizational, leadership, and coaching skills.
  • Ability to manage a remote team independently.
  • Excellent communication and problem-solving skills.
  • Preferred: Experience in Chronic Care Management (CCM), Transitional Care Management (TCM), or Remote Patient Monitoring (RPM).

Work Schedule

  • Full-time, salaried position (40 hours per week, typically 9–5:30 EST, Monday to Friday).
  • Must commit to at least 1 weekend shift per month.
